There is no federal template, so employers and schools accept any note that covers the basics. Use this checklist for a doctor's excuse for work or school.

Keep the medical detail minimal. Under the HIPAA Privacy Rule, health care providers are limited in what they can disclose, and the rule does not stop an employer from asking an employee for a doctor's note. How to write a doctor's note step by step
- Confirm who the note is for. Use the patient's full legal name as it appears on employment or school records.
- Record the visit. Note the date the patient was seen or treated. The full clinical record stays in the chart, not in the note.
- State the dates and status. Give the start and end of the absence and say whether the patient is off work, on modified duty or cleared.
- List restrictions clearly. Use specific limits, such as no lifting over 10 pounds for two weeks, rather than vague phrases like light duty.
- Share a diagnosis only with the patient's agreement. Keep the note to what the employer or school needs.
- Sign, date and add credentials. Put it on letterhead with a phone number for verification.

How long is a doctor's note valid?

Do I need a doctor's note for one sick day?
