Choose the most specific site documented: unspecified part of neck (S72.001A), intracapsular or subcapital (S72.011A), midcervical (S72.031A displaced, S72.034A nondisplaced) or base of neck (S72.041A displaced, S72.044A nondisplaced). A closed displaced fracture of right femoral neck with no further site detail usually codes to S72.001A, because the unspecified neck code has no displacement axis; if the provider names the midcervical or basicervical region, use S72.031A or S72.041A.
Pertrochanteric fractures (S72.1-) include intertrochanteric (S72.141A displaced, S72.144A nondisplaced) and isolated greater or lesser trochanter fractures. Subtrochanteric fractures, just below the lesser trochanter, use S72.21XA (displaced) or S72.24XA (nondisplaced); the X is a placeholder so the 7th character stays in position 7.

Code the fracture first, then the external cause. For a right hip fracture from an unspecified fall, report the fracture code (for example S72.001A or S72.141A) followed by W19.XXXA, unspecified fall, initial encounter. Use a more specific W00-W19 code when the type of fall is documented, such as a fall on the same level from slipping or tripping. External cause codes use the same 7th character logic and are never the first-listed diagnosis.
Outpatient and home health PT after a right hip fracture is usually a subsequent encounter, so the 7th character is D (routine healing), G (delayed healing) or K (nonunion).
