Carelon Prior Authorization Form Step-by-Step Instructions
1. Download the right form
Download the Carelon request form or open the request in the portal. Use the fillable PDF if you must fax, because it removes handwriting errors. Before faxing, check your plan's page for a newer version, since the PDF carries no date.
2. Decide the submission route
Use the portal when your plan offers it. Call for urgent requests, as the form instructs. Where a plan restricts fax to existing requests (BCBS Texas) or requires electronic submission (Michigan commercial), a fax will not start a new request.
3. Enter subscriber and patient details
Use names exactly as on the insurance card, the full member ID including any prefix, date of birth in MM/DD/YYYY, the relationship to the subscriber, plan name, group number and product type (PPO, POS or HMO).
4. Add referring physician details
Include the physician's full name, specialty, current phone and fax numbers, and practice address with suite number.
5. Add provider and facility details
Give the name, contact numbers and street address of the facility where the service will be performed.
6. Describe the procedure

Carelon prior authorization for physical therapy
Several Blue plans run therapy authorization through the Carelon rehabilitation program. It covers physical, occupational and speech therapy, and the Blue Cross NC notice states that authorization requirements are based on the CPT code filed. Blue Cross NC applied the program to fully insured commercial members from December 2024 and to ASO groups from July 1, 2025, and excludes autism spectrum disorder diagnoses.

2026 prior authorization rules: what changed and who is covered
The CMS Interoperability and Prior Authorization final rule (CMS-0057-F) sets new requirements for specific payers. The CMS fact sheet lists them as Medicare Advantage organizations, state Medicaid and CHIP fee-for-service programs, Medicaid managed care plans, CHIP managed care entities and Qualified Health Plan issuers on the federally facilitated exchanges. It states that commercial and employer plans are not covered, and the rule does not apply to drug prior authorizations.
